Health Requirements

Physical Examination

All students entering Pre-K, Kindergarten, and Sixth Grade, or any new students to Fairmont are required by the State of Illinois to have a current physical examination and all required immunizations, by a licensed health care provider. School physical forms are due by October 15th of the current school year. Your child will be excluded from school on October 15 of the current school year until a school physical has been completed and submitted.

Dental Screening

All students entering Pre-K, Kindergarten, Second, and Sixth Grade are required to have a dental examination before entrance into these grades (by May 15th of that school year).

Vision Screening

All students enrolling in Kindergarten and any student enrolling for the first time in an Illinois school must have an eye examination. Parents must provide proof that the child was examined by a physician licensed to practice medicine in all its branches or a licensed optometrist within the previous year before October 15th of the school year.

Medication Authorization

District policy states that medication may be given to students only upon the written request of the student’s physician and parent. The medication authorization form must be completed and returned to the school nurse before the medication can be given. It must be updated every school year. Unclaimed medication at the end of the school year will be discarded.
