State of Illinois Health Requirements for School Year 2024-2025

In anticipation of the school year 2024-2025, the following information summarizes our student’s health requirements. Please submit all the health requirements before the first day of school. All the required forms can be downloaded from the Fairmont website.

Requirements by Grade Level:

Preschool Students:

• Physical Exam - Parents/Guardians, please complete the HEALTH HISTORY on top of the form then sign and date the form

• Up to date Immunization

• Lead Questionnaire or Lead Screening

Kindergarten Students:

• Physical Exam - Parents/Guardians, please complete the HEALTH HISTORY on top of the form then sign and date the form

• Up to date Immunization

• Dental Exam on IL form

• Eye Exam (done by an optometrist or ophthalmologist) on IL form

1st Grade:

No health requirements if up to date. Otherwise, you will be receiving notification on missing health requirement/s.

2nd Grade Students:

• Dental Exam on IL form

3rd, 4th, and 5th Grade:

No health requirements if up to date. Otherwise, you will be receiving notification on missing health requirement/s.

6th Grade Students:

• Physical Exam (Sports physical is NOT acceptable, IL Certificate of Child Health Exam form is required) Parents/Guardians, please complete the HEALTH HISTORY on the top of the form then sign and date the form.

• Up to date Immunization

• Dental Exam on IL form

7th and 8th Grade:

No additional health requirements if up to date.

• Tdap &/or Meningococcal Vaccine if it has not been received during 6th grade.

• Student Athlete - Preparticipation Physical Evaluation Form needs to be submitted. The History Form must be completed by the student.

Students Requiring Medication in School:

Please note that each medication or treatment requires its own form. If your child takes 2 or more medications/treatments to be self-administered or taken at school, you will need to complete a form for each treatment or medication.

• If your child requires medication or treatment (prescription or over-the-counter medicine) to be taken in school, please submit the Medication Administration Authorization Form properly completed. This form needs to be renewed yearly.

• If you want your child to carry and administer their own emergency medication or treatment, such as an asthma medication, diabetes medication/treatment, seizure medication, or epinephrine injection, please submit the Self-Administration of Medication Form properly completed. This form needs to be renewed yearly.
